How to Request Accommodations

Steps to receive accommodations:

  1. Complete the New Student Information Form
  2. Submit documentation from an appropriate professional source
  3. Meet with the Coordinator of Student Disability Services
  4. Collaborate to develop an individualized Accommodation Plan
  5. Receive and use your approved accommodations

Eligibility & Documentation 

To be eligible for services:

  • Submit appropriate documentation of your disability
  • Send documentation directly to Disability Services (not Admissions)
  • Documentation is kept confidential by the Coordinator

What happens next:

  • Documentation is reviewed
  • You will be contacted to schedule an appointment
  • Accommodations are determined through an interactive, case-by-case process

Important: Students may request accommodations at any time during their enrollment.

  • Share your Accommodation Memo with your instructors each semester
  • Communicate your needs in a timely manner
  • Submit testing requests in advance when applicable
  • Contact Disability Services with any issues or questions
