The Student Health Center is committed to creating a seamless transition through meeting your medical requirements, so please review the information on this page very carefully.

We have partnered with Med+Proctor for verification of your immunizations and medical requirements for Missouri Southern State University. All students new to the University, must use Med+Proctor for collection and verification of health information. This includes new, transfer and former students returning. (International students are the only exception as they will be completing requirements with the Health Center assisting them during International check-in week prior to school starting.)

Med+Proctor is a secure, cloud-based, fully automated system and user-friendly platform that is both HIPAA and FERPA compliant. With this format, you can even upload clear pictures straight from your smart phone.

Medical History Form

Medical History Form is the only requirement NOT submitted and tracked in Med+Proctor.

A Medical History Form must be submitted to the Health Center before the semester begins. Detailed medical requirements and instructions are listed on this form. You can pick this form up at the Health Center or find it on our website. Complete front and back, and return the form to the Willcoxon Health Center by bringing it to Southern Welcome, mailing, faxing, or dropping it off at our office. Emails are not accepted.

MED+Proctor: Vaccine Requirement & Tuberculosis (TB) Risk Assessment

Use your MSSU email address to make an account in Med+Proctor. All students will upload proof of 2 Measles, Mumps, Rubella (MMR) vaccines or an immune MMR titer and complete a TB Risk Assessment. (If born prior to 1957, no proof of MMR is required.) If you are living in campus housing, you will also upload proof of your Meningitis (MCV4) vaccine age ≥16yrs prior to moving in.

When a communicable disease comes to the attention of the University, whether student, faculty, or staff member, the University's physician and nurse will assume the responsibility for conducting a thorough review based upon the best medical and legal information available. Any action taken will respect the confidentiality of the individual as well as the individual's welfare and that of the University community.
