You should download a complete printable version of the Application Form to be completed for the IRB and then submitted in paper form with appropriate attachments and signatures.

The IRB numbering scheme is detailed below:

Number Scheme (aaaa-b-cccccc-ddd)

— a. Department of Researcher (four letters)
— b. Student (S), Faculty (F) or Graduate Student (G) Researcher (one letter)
— c. Year/Semester-Banner style (four/ two digits)
— d. Sequence Number (three digits)

Number example:

NURS-S-200680-001 would indicate a research application from an undergraduate student in the Nursing Department during the fall 2006 semester. This would be the first application from that department for this semester.
