COMPREHENSIVE DRIVER SAFETY PROGRAM

RE-CERTIFICATION TEST

In order to maintain DOT certification, drivers must be re-certified every two years. Re-certification requires the employee to submit to a new physical examination and drug screening. Re-certification testing must be conducted prior to each biennial anniversary date in order to prevent a lapse in certification. The Human Resources Department will schedule all testing for the employee at an approved DOT testing facility.

After successful re-certification testing, the employee is provided with a newly updated Medical Examiner s Certificate. Employees who fail the physical exam may be reassigned as necessary on a temporary or non-temporary basis. Employees who fail the drug screening are subject to disciple up to and including termination.

Re-certification testing is preformed biennially, once very two years, regardless of any other required testing to which the employee may have recently submitted.

A copy of the re-certification shall be given to the Human Resources Department for verification and shall be kept in the employee s DOT file.
