Common Data Form Instructions

We are at present converting to an electronic processing of information via the Financial Aid office at the college. You may now mail , e-mail, fax, telephone, print the Common Data Form, or send it via the World Wide Web.

You need not complete the entire form however; but there must be sufficient information included to see that it is forwarded to the right department or person(s); and to include all the necessary information you are attempting to communicate. You may use the box at the end to type in a text message that will then be sent to the department or person(s) you wish to contact.

Our Common Data Form has been designed to provide and update information to a variety of offices, services, and personnel on campus. Such as:

Anyone may use our Form: faculty, applicants, students, high school counselors parents, guardians ...

Your message will be forwarded electronically to the appropriate office on campus. In time they will either respond, or they will complete the appropriate action for you (Ex: updating, amending, correcting).
