Medical History Form

Immunizations

A Medical History form is required for all Missouri Southern Students. Please complete the form and send it to MSSU's Health Center or bring it with you to Southern Welcome!

During the Summer Southern Welcome session students have the opportunity to receive the MMRII and meningitis vaccinations.

(2 doses of MMRII are necessary and the Meningitis vaccination is required of all residence hall students).
